If you wish to register with one of our GP's, please call into the surgery bringing your National Health Number with you.
We also require two forms of identification - preferably one form of photo ID and an address verification (not required for children living with parents/guardians)
One of our Reception Staff will then give you some registration forms to complete and offer you an appointment with the Healthcare Assistant for a New Patient Check.
The New Patient Check is a basic health check where the nurse will check your Blood Pressure, height and weight, and ask if you have any health problems that your Doctor should be aware of, or if you have any family history of certain diseases that could be of importance to your future health.
This is not compulsory but in an event of an emergency it is helpful to have these details.
If you are on any medication you will be offered an appointment to meet and discuss this with your registered GP.
After completing the registration forms we will inform the Health Authority of your new details and your notes will be retrieved from your previous GP.
