Information & Contacts Home Visits Home visits can be very time consuming for the doctors and other patients. They are for patients too seriously ill to come to surgery. Patients with a rash or temperature may still come to the surgery and will be seen more quickly this way. Please make requests for home visits before 10:00am giving the patients name, address, telephone number and a summary of their medical problem. Nights & Weekends If you need to contact a Doctor in an emergency, you can contact the Out of Hours Service which is stationed in Drayton. Their contact number is (023) 9237 7921 and the service is covered by GP's f rom the surrounding areas. Or NHS Direct for medical advice on 0845 46 47 Sickness Certificates For any illness lasting seven days or less you do not require a doctors sickness certificate. However, your employer may require you to complete a self - certification form (SC2) which you can obtain from your employer. Any illness lasting longer than seven days will require a visit to your doctor for him/her to issue a sickness certificate. Access for the Disabled The surgery is at ground level improving access for the disabled. |
